Payment options include monthly payments or payroll auto deductions with the cooperation of your employer but it is always the defendant's responsibility to make sure payments have been withheld by checking their paycheck stub. If payment has not been withheld, you will need to make the payment(s) yourself. 

Download/Print the Payment Plan Questionnaire HERE and email this form to Fine Collections Office. If you are unable to print this form, EMAIL the office and request that a form be sent to you by mail.  If you are unable to email the form once completed, you will need to mail the form to the office.
